All athletes MUST have the following completed BEFORE they can participate in athletics:

1) A physical and medical history form- see bottom of page for form

2) All athletes need to complete the REQUIRED online athletic paper work

Electronic Forms include: 1. Athlete Info; 2. All UIL required Participation Forms; 3. Medical History Form (New Online).

*** In order to participate in Athletics, athletes are REQUIRED to purchase the athletic t-shirt we provide and a lock. The fee will be $15. Black bottoms can be purchased on your own.
