Prevvy delivers a personalized patient engagement experience by supporting dynamic, automated care plans. We incorporate the latest health interoperability standards (FHIR, SMART on FHIR), and Patient-Centered Clinical Decision Support (PC-CDS) based on evidence-based CQL specifications that allow the definition of detailed dynamic Care Plans and Patient Recommendation protocols for specific populations. Our solution is disease-agnostic and incorporates multiple remote care services, allowing both patients and care teams to rely on a single platform for patient engagement and patient communication outside the clinic.
