A pre-authorization is necessary for me to bill your insurance company directly. 

Please ensure that you bring your insurance information, including the policy number and authorization code, to the first consultation. 

If pre-authorization has not been obtained before the consultation, you may be required to make payment on the day of the appointment; and subsequently submit the invoices to your insurance company for reimbursement.

Any excess deducted from your insurance policy will be billed at a later date. 

Self-funded patients are welcome into the private sector; preferrable with a referral from GP or another Specialist, although this is not an absolute pre-requirement. 

In line with my duty to the continuity of your care, I endeavor to communicate the outcome of your Consultation to your GP unless you explicitly request otherwise in writing. In this scenario, the record of the consultation will be stored only in the hospital.

The invoice for self-pay outpatient services will be issued after the consultation and treatment. Kindly ensure that the necessary actions are completed by the specified deadline. 

Specific procedures required during Consultation might incur additional costs for which you will be notified beforehand.
