How Do I?

Request a transcript

If you would like to request a copy of your student transcript, please complete and submit the Request for Transcript Form with a non-refundable transcript fee of $20 (cash or money order). Requests can be emailed to cosbyk@hdsb.ca

Proof of identification is needed when picking up transcripts.

Drop or Change a Course

You can either book an appointment with a guidance counsellor or email them with the request. All changes will require a parent / guardian signature (if 18 & under). The forms and appointment booking are in the section above.
