The work permit has three sections that must be completed.

1. Student Information - This must be completed with the applicant's information. 

2. Physician - This section includes an up-to-date physical examination.  Must be completed by your physician. 

3. Pledge of Employer - This section must be completed by the hiring employer.  

4. Take the completed Work Permit to your school’s front desk for final approval.
