Audition Agreement Form

Directions: Click on google document below. Print form so both dancer and parent can sign. Forms must be turned in in paper format and signed with pen. If you do not have access to a printer, a few forms will be available in the dance studio before the due date. 

Dancers must turned in signed Audition Agreement form before or at the check-in process. Dancers will not be allowed to audition without a signed Audition Agreement form at check-in

Forms may be submitted to Mrs. McWithey any day before March 12th (recommended)

**Your signature indicates that you have reviewed the audition process and have read through the entire audition process and understand that being a member of the Hammond Dance Company requires a great deal of time, dedication, and commitment on the part of my daughter/son. I am also aware that should my daughter/son be selected to the Company it may include certain transportation needs, fees for festivals, and purchasing basic dance gear or shoes. Specific performances beyond the school day are required.**
