Physicals and Final Forms

If you played a sport this school year, even though it says your physical expires in April, HCS and Erlanger have waived that expiration to the end of the school year.  Proof of a CURRENT Physical (Final Forms) should be provided prior to Tryouts.  

A current physical for the 2025-2026 school year must be on file in Final Forms by August 8th in order to play. Players are encouraged to see their regular provider and get the physical completed anytime after April 15.

Upload your physical into Final Forms and bring me a physical copy please.
