HSD Connect Enrollment for the 2023-2024 school year is open!

E-forms are now being accepted for students in grades 1-5 to attend Hazelwood Connect, Hazelwood School District's virtual learning school for the 2023-2024 school year. This option is available to any student who is enrolled in the Hazelwood School District in grades 1-5.

Steps to Enroll in HSD Connect Elementary Virtual Academy:

• • • Student must be enrolled in the Hazelwood School District. 

    • Complete the HSD Connect Virtual Academy optional e-Form on Parent Portal. 

    • Click here to be redirected to the Parent Portal to complete the optional e-form.

    • Change the Academic Year to 2023-2024. Then select View "New Optional e-Forms are available" to complete and submit the form for review.

If you are not currently enrolled in the Hazelwood School District, you will need to first enroll in the district and then complete the MOCAP e-form. Click HSD Student Enrollment  for more information.

Once district enrollment is complete, please complete the applicable optional e-form in the Parent Portal to request virtual instruction. 

Students approved to enroll in Hazelwood Connect will be required to remain in Hazelwood Connect for the entire year.

Enrollment in Hazelwood Connect Virtual Academy will be limited and contingent upon enrollment. If enrollment for Hazelwood Connect has reached capacity and/or if enough enrollment is not met, the district's Coordinator of Virtual Programming or your child's Building Principal will reach out to you.
