This partnership program allows us to have a mental health therapist and case manager from Horizons Mental Health Center within our school district, and a coordinator to help with the program.
Does your child struggle with stress, anxiety, anger, sadness, or behaviors that make it difficult to stay in class? Maybe there are big life changes going on in your family right now. If your child does not have behavioral health services but you feel like it would be something they could benefit from please let us know by filling out the form or emailing our coordinator. If your child is already receiving services and you would like to have options for services in the school from Horizons Mental Health Center please contact our coordinator.
The nice thing about the Caring Team Program is your student will not have to leave school for their appointment. The therapist will have an office at the school. Intakes and appointments will be delivered in the school, and you will have access to our coordinator (located in the school) who will help connect parents/guardians with Horizons Mental Health Center. This includes help with scheduling and acting as the connection between the parents/guardians, teachers, and the therapist/case manager. We strive to give our students the best care, most productive school experience, and help families connect to support services in a convenient and local environment.
Due to wanting to create sustainability for the Caring Team Program and to be able to provide this service for years to come, Horizons Mental Health Center will be billing for services. They will bill either Medicaid insurance, private insurance, or discuss other financial options and covered services at the intake appointment.
If you wish to contact Horizons directly they can be contacted at: 620-663-7595, let them know you are part of the Haven District and are interested in school based services. The crisis line is 1-800-794-0163. If you wish to visit their website and see more resources it is:
