Information for filling out Textbook Condition Contract

THE TEXTBOOK CONDITION CONTRACT


Please fill out the form to note any pre-existing damage to the items issued to you. It is to your benefit to do so. Any pre-existing damage of the textbook not reported on this form is subject to fines. Protect your books from any damage. Keep them away from small children and chewing pets!


Yearbooks will be held at the end of the year until all textbooks, library materials, and patron fines have been cleared. IF DAMAGES ARE NOT NOTED ON THIS FORM THEN FINES WILL BE ASSESSED AT THE END OF THE YEAR.

Any consumable textbooks are to be used and written in by the student. Once books are issued to each student they are NOT returned to the library.

1 Science (7th=hardcover book, 8th=consumable)

1 Math (7 & 8, Algebra=consumable, 7th Accelerated=hardcover book)

• Use this list of possible damages to help you describe the condition the book was in before you got it. 

• Fill out the Google Form documenting the condition of the home set of textbooks. Don’t worry about the monetary assessment.

• Enter the barcode number that says “Arroyo Seco” or another Hart District School

• List any problems where it says “description”.
