A. Immediate dates and information:

□      By 10/22: For text blasts from the coaches, register for remind.com:   

□      By 11/3: Complete forms online at athleticclearance.com

□      By 11/3: Submit annual physical form (with physician) / or note with upcoming date of exam

□      By 11/3: Emergency release form--HARD COPY IN PERSON please

□      By 11/3: Commitment Contract—discussed and signed

□      Please consider having your student buy an ASB card: $75 at the ASB Office    

□      Do you have individual or family health insurance? No? Ask us about the district’s accident insurance.

□      Please consider the team’s requested donation of $250 or $350 per family. (Note: ASB’s online fee.)

□      Team gear orders: The team stores CLOSE ON NOVEMBER 3. Links at saugusswim.com.

PLEASE buy each swimmer a team suit and polo shirt. This is optional but preferred.

Swim caps: Coaches bulk order these. 

Latex: $5 / Silicone $15. Order anytime – link at the top of saugusswim.com 

Bring a coach a receipt from A.S.B.
