If you have multiple students coming to LA MESA for the 2025-26 school year,

please complete the process for each student:

The following list MUST be completed before you attend your Summer Check-In Day!

🔲 Step 1 - Immunizations - MANDATORY

If your child is missing any required vaccination, the school's health technician has already sent you a notification. If you have received a notice from the school regarding any missing vaccines, please ensure all vaccinations are up-to-date ASAP.

*IF YOUR STUDENT ARRIVES ON THEIR CHECK-IN DAY WITHOUT THESE ABOVE ITEMS COMPLETED, THEY WILL BE ASKED TO RETURN HOME TO HAVE PARENTS COMPLETE THE FORMS OR PARENTS MAY COME WITH THEIR STUDENT ON CHECK-IN DAY TO COMPLETE THE FORMS FROM A SCHOOL COMPUTER, WHICH WILL DELAY YOUR STUDENT'S CHECK-IN DAY.
