At the end of each academic unit or certification course, students shall complete and turn in an “Course Evaluation” form. The results of the evaluation forms will be compiled by the Adult Education Coordinator or the Coordinator of the PN program and forwarded to Assistant Director or his/her designee

At the conclusion of each clinical rotation in the Practical Nursing Program, students shall complete and turn in an “Evaluation of the Clinical Experience” form.

Immediately after graduation from the Practical Nursing program, students shall complete and turn in an “End of the Year Evaluation” of the practical nursing program. The completed program evaluation forms are completed online and not shared with anyone until after the class graduates.

Personnel from each clinical facility will be asked to provide input by completing an evaluation form regarding the clinical learning experience.
