Forms

We believe that sharing some of your observations in writing is important and can be a lasting resource for the colleague which you have observed. We respectfully ask you to do so in a timely manner after your observation. With that said, those observations are a personal conversation and are to be kept between the two of you. Similarly, the form that those observations notes take - should work for you and your partner - thus we are not requesting a specific format be used.

Simultaneously, we acknowledge that not providing you with a form although freeing, also adds to your "to do" list - that is not our intention and for this reason we are sharing a couple of examples we've found below. If they work for you, great! use them; if they do not, please feel free to modify them in a way that will work for your growth.

Finally, if you have a form that works for you - it might work for a colleague outside of your partnership. If comfortable, I'd love to add it to the list of examples shared on this site.
