Email Ms. Mattingly [jennifer.mattingly@guhsdaz.org]
or stop in the counseling office to discuss completing the required testing
4) Submit Application:
Follow these steps to fill out the application:
Open Application
Download and save the application to your computer
Close the current document and open the one you just saved
Fill out entire application [signatures can be typed]
Save
Email or drop off the application to Ms. Mattingly by January 9th
5) Score a minimum of 60/100 points on the applicant rubric based on:
Attendance
Discipline (Students are ineligible to apply if suspended due to drugs/alcohol/smoking)
GPA
Math Exam Score
Reading Exam Score
