Any student who wishes to participate in a sport must provide the following:

Completed physical packet with parent signature which includes emergency information, medical insurance policy number or purchased school insurance, and a parent signed mild traumatic brain injury statement each year.  Athletes must also complete and pass the concussion course once while at Cortez.   Freshmen will be offered this course during their P.E. class.  http://aiaacademy.org/brainbook
