Welcome to the Online Clearance Process Guide. Here you will find the directions to complete the clearance process to participate in athletics and activities. This system has been used since the 2015-16 school year. Thanks for taking the time to complete the process.

Online Clearance Portion

Online clearance must be completed for each sport you intend to participate in this year.

PLEASE READ ALL STEPS BELOW THEN PROCEED

Step 1

Go to homecampus.com

Step 2

Complete the Log-in Process.

Step 3

Choose as many sports as you think you will try/play.

Step 4

Complete the Online Signature Agreements.

Step 5

Print and Sign the Confirmation/Consent Form. Print from the site or from the email that is sent to your registered email.

Step 6

Bring the signed form (or email) and your original current physical to the Finance Office*

* – If purchasing Student Insurance, please turn in a copy of the printed ID card to finance.

Physical Form Portion

Physicals are required annually and must be valid throughout the entire season of your sport. Please have Doctor or Medical Provider stamp and sign the completed Physical form.

Step 2

Complete the top portion prior to your doctor’s visit.

Step 3

Have a licensed healthcare provider complete the bottom portion of the form.

Step 4

Make a copy for your records

Step 5

Bring your original current physical and the signed clearance/consent email to the Finance Office.

You may now clear for multiple sports with your up-to-date physical. Please choose as many sports as you think you will play at the beginning of the clearance process. If you decide later to try a new sport, see finance to add a sport to your current clearance. You will be given a new confirmation letter to be signed and returned to finance for clearance.
