Welcome to the GSDMSFI AY 2025-2026 Enrollment Portal. Here you can check the step-by-step process of your enrollment and the requirements and procedures for each step. We are hoping to see you here Future Dokista!
Step 1: Online Application
Access the AIMS Portal.
Create your AIMS account.
Save your username and password for login and status checks.
Complete the Google Form and note your chosen exam date. Once submitted, your exam appointment is automatically confirmed.

No AIMS Account, No Exam.

GSDMSFI SHS graduates and previous applicants with an AIMS account do not need to reapply.

Step 2: Entrance Exam and Interview
In-Campus (GSDMSFI):
Report to the Guidance Office by 8:00 AM with:
1 pc. 2x2 or passport-size photo
1 photocopy of school ID or any valid ID
Mongol #2 or HB pencil
Black ballpen
Notes:
The exam starts at 9:00 AM. Latecomers will be rescheduled.
Interviews are usually done in the afternoon. If not, passers may join any posted interview schedule on the official GSDMSFI Facebook or Guidance Office page.
Off-Campus:
Arrive at the venue 1 hour before the exam with the same requirements as above.
Notes:
Exam starts at the agreed time. Latecomers will be advised to take the exam at GSDMSFI.

Step 3: Medical Examination
● After passing the admission interview (reflected in your AIMS account), undergo the following tests:
1. CBC
2. Chest X-Ray
3. Drug Test
4. Stool Exam/Fecalysis
5. Urinalysis
6. Serum Pregnancy Test (for females)
7. Hepatitis B Screening (HBsAg)
8. Hearing Test (if recommended during the interview)
9. Other tests or medical certificate as required during the interview

Medical exams should be done at GSDH Medical Arts Building OPD Laboratory (items 1–7).
Requests for medical exams (discounts will apply) are available at the school clinic.
Submit original medical results to the School Clinic before document submission.

Step 4: Document Submission
Incoming First-Year Students:
Grade 12 Report Card
Certificate of Good Moral Character
PSA Birth Certificate
Transferees:
Transcript of Records (TOR) – Original
Honorable Dismissal – Original
Certificate of Good Moral Character
PSA Birth Certificate
Where to Submit:
● Guidance Office: Photocopies of documents + 1 pc. 2x2 photo (long brown envelope)
● Registrar’s Office: Original documents + 1 pc. 2x2 photo (long brown envelope)
Step 5: Encoding of Subjects

Step 6: Payment
● Minimum down payment: PHP 1,500.00 (for freshmen and transferees only).
● Pay at the Finance Office, through bank partners, or via QR code (GCash, Maya, PayPal).
After payment (For online payments and bank deposits only):
Take a clear photo of your deposit slip.
Email it to gsdmsfinance@gmail.com with the subject: (STUDENT NAME) PAYMENT (AMOUNT).
Include the following in the email body:

Amount
Bank

If you’re truly interested in enrolling but did not pass the entrance exam, don’t worry! You can still write a formal letter to the Program Head of your desired course (except for BS Nursing) explaining your intent to enroll. Please include a copy of your grades with the letter and submit it personally.
Kindly note that your grades will be reviewed by the Program Head, and we’ll keep you updated through your contact details. An assigned staff member will also reach out to you once we have a decision.

We Also accept Government-Initiated Scholarships like CHED, EDUKAR, OWWA, KABUGWASON, and etc., but you will be applying directly to their respective offices.
For Unifast-TES scholarship, application will start during first semester which means you must be officially enrolled before you can apply. Unifast-TES application is subject to CHED verification and approval.
