Make a copy of this and make sure it is in your folder under essential documents Faculty Buddy folder. Some teachers like to fill this out electronically so you will send them the copy you make so they can edit it directly. Some like to fill it out in pen so you will need to print a copy and bring it to your meeting. I will print copies and have them available to you when I return to school, however, if you meet before then you will need to print it out yourself.
