Athletic Forms
All forms must be complete and submitted to the head coach of the sport the student is participating in BEFORE they begin any type of participation with the team.
A copy of the athlete's birth certificate must be submitted as a part of the required documents. This only has to be done once during the athlete's career at Southside High School.
