The OnTrack Facilitator is ultimately responsible for managing and organizing all routine meetings, setting the stage for the success of student interventions.
Specific duties include:
Prioritize flagged students using EWRS data and other resources, referrals
Collaborate with core team member/s to review and prioritize referrals
Prepare for OnTrack meetings by collecting and summarizing student data, context, and background information
Develop and send agenda (including student list) to team members at least two days prior to the meeting
Ensure all meetings start and end on time
Encourage and facilitate team discussion of root cause analysis and link to interventions
Assign advocate and follow-up date
Ensure timely and accurate data entry into the Intervention Connection System
Communicate and follow-up meeting outcomes with the principal, absent team members, and stakeholders
