ReTake and NHI

Take Two Student Request FormThis form is intended to allow parents and students to request a Take Two on any MAJOR assignments. Please fill in all questions on the form. Once received, your teacher will contact the student regarding the required re-learning opportunity and a Take Two date. Students may request a retake, redo, or revision of an eligible major assessment. To qualify, the student must: Submit the teachers Retake/Redo Request Form within five school days of the grade being posted. Have submitted all minor assignments associated with the instructional unit. Complete teacher-directed remediation or relearning before reassessment. Coordinate the reassessment with the teacher during an agreed-upon intervention time. Student can only retake Major assessments once. The highest score will be recorded in the gradebook.
