All field trips must be approved by Ms. Simmons PRIOR to collecting any money or communicating with parents. If money is being collected, have all applicable field trip forms to Mrs. Shannon at least 3 weeks prior to the date you would like to begin collecting money. If no money is being collected, please be sure to explain this to Mrs. Shannon and have all applicable field trip forms (excluding permission slips) handed over at least 2 weeks prior to the date of the trip.

Google Maps mileage print out for all field trip stops, if using GCS Buses
Completed PO's for any field trip expenses (district/charter transportation, substitutes, tickets, food, lodging, etc.) - quotes must be attached for all expenses except GCS bus and substitute costs. PO's will be available in Mrs. Shannon's Office or can be requested via email to be delivered to your mailbox. Only one form is necessary as long as all expenses are in their own line and all quotes and invoices are attached.
Copy of any communications sent home to parents.
Please be prepared to provide a copy of your detailed field trip lesson plans and instructional plans for non-participating students, should Ms. Simmons ask to review them.
*NEW THIS SCHOOL YEAR* PLEASE do not copy all of your signed student permission slips to turn in to me. Instead, please scan these to email me a digital copy. Physical copies are not required for my field trip packet that I bookkeep. Please continue to hold your copies for your records.
