This form allows you to nominate a deserving caregiver for a transformative experience that emphasizes the importance of putting their well-being first.

Who Can Be Nominated? Eligibility Requirements:

1. Reside in Colorado

2. Be a Parent or Caregiver of One or More Chronically Ill or Special Needs Children

3. Be Available for a Minimum of 2-3 Nights During the Offered Trip Dates

4. Be Able to Arrange Care for Your Child/Children While You're Away
