If a student is selected for a team they must turn in an acknowledgement of risk form and consent for emergency care form in order to be eligible to travel with the team and compete. Forms may be downloaded below, or can be picked up from Coach Pappas office, and returned to their sport specific coach. Any questions may be directed to your sport specific coach or Coach Pappas
