To participate on the Kearns High Swim Team You must complete the ELIGIBILITY PROCEDURE:

KHS Fees and Forms

All Fees go to the Bookkeeper at the high school.

1. Swim Participation Fee – $154 unless on fee waiver. This fee includes transport fee, team suit, T-shirt, etc.

3. ALL SWIMMERS MUST GET A PHYSICAL Physical Form On File (Form A) - Physical Form give me a copy when you are done.

You will be expected to follow the UHSAA Eligibility Rules. In addition to the rules that govern alcohol and tobacco, you have to be academically eligible. You are in school first and foremost to learn. Swimming and Water Polo comes second to your education. Preserve your eligibility by good attendance, and getting your homework in on time.

Refunds: Students who choose to participate in athletics must realize there are no guarantees regarding playing time or injuries. No participation fees will be refunded after the day prior to the first scheduled meet. A participant dismissed for a disciplinary reason will not receive a refund. Refunds will be only considered for “special” circumstances (i.e., family situation, etc.) and request must be made to the Principal.
