If your child has a food allergy, an Action Plan AND Dietary Accommodations form must be filled out by a physician.

The medication must be in its original container with the pharmacy label and must not be expired. The pharmacy label must be affixed and include the student's name, doctor's name, name of medication, dosage and times for administration.
Over the counter medication (ie. Acetaminophen or Ibuprofen) will require a doctor's order (Physician's Request... form).
The parent is required to drop off the medication that is to be given to their child at school. Remember, for the safety of all children at school, students may not carry any medicine (even cough drops) with them while on school grounds.
Any medication that needs to be returned home must be picked up by the parent unless other arrangements have been made with the nurse.
Medications left at the end of the school year will be disposed of appropriately.
