GPISD requires physicals to be updated every year. Every student must turn in a new physical at auditions. On the tab below you will find a copy of the physical form, which must be filled out by your physician. A hard copy of these forms is available in the athletic field house.
In addition to the physical form, each student must have the GPISD Emergency Card found below. Please email vboyce@galenaparkisd.com for a form that can be filled out digitally.
