Golder Eligible
To be Golder Eligible, students must have turned in all of their required forms:
Class of 2027 Forms - 9th Graders
Physical Exam & Immunizations Record - 9th Graders and Athletes
Dental Paperwork - 9th Graders
Family Income Information Form - All students whose parents received a form via email.
Media Release Form - all students need to submit this yearly
2 proofs of address - all students need to submit upon enrolling or making changes to mailing address
Proof of being Medically Compliant - Juniors and Seniors after turning 16 years old need a second dose of MCV4
NCP - Acknowledgement of receiving the Student/Parent Handbook - All Students need to submit this yearly
General Permission Slip 1 Mile Radius - All Students need to submit this yearly
