All forms are printed and sent home during the first parent meeting of the season. If you are in need of another copy, please feel free to print and return to the corresponding coach. 

All forms must be submitted prior to the first day of practice. Failure to do so will result in the limited participation of the student-athlete. 

These forms are to be seen and acknowledged by both the student and the parent/guardians of said student to have a complete understanding of participation in Golden Rule School's Athletic Programs.
