PCDM Clinical Proficiency Requirements

Any Doctor performing Botox or filler injections in the office must: 

• Maintain a clear and accurate account of injectable products in stock

• Maintain accurate records, including pre-treatment diagnosis and documentation in the patient record of locations, dosages, expiration dates, and lot numbers used in patient care. Any waste must also be documented

• Utilize and sign approved patient consent forms that are to be uploaded into patient record

• Utilize appropriate financial consent forms prior to treatment

• Provide evidence of completion of an accredited course that covers: 

• An overview of Botox and other products 

• An overview of the FDA-approved Botox procedures and the underlying conditions associated with them. 

• An overview of facial anatomy 

• Best practices for safe Botox administration 

• Procedure-specific guidance for Botox use 

• Procedure demonstrations and hands on instruction
