Training and support for school staff on health related concerns

• Support connections to Health Services in local community

• PD sessions on general health conditions in schools

• Provides current, evidence-based information about health diagnosis and treatment options

• Supporting Schools to interpret policy documents related to health care plans

Linking school staff into community resources

• Advocate for students with Health needs

• Liaison between clinics, Primary Healthcare Providers, and Pediatricians, school, and family

• Maintaining student and family confidentiality

• Identifies gaps between health and education and works with interdisciplinary teams to close them

• Support PD to school based leadership in process and procedures for health care plans

• Creating an individualized Health Plan of Care

• Liaison between clinics, Primary Healthcare Providers, and Pediatricians school and direct Student Health Care Plan

• Integrate outside healthcare connections to support students with complex healthcare needs

• Create student specific forms, logs, EA supports

• Direct training for Special Transportation Drivers
