These devices are provided to teachers and students for use at home. You must provide your own internet. It is not included with the device.

After you have completed the form, you can scan or upload the form or you can take a picture of it.
This is what you will attach to your application.

This is an online form. The last question will ask you to submit the Device Agreement Form. This is where you add the form from Step 1 above.
