Answer each question to indicate whether you need lift tickets, rental equipment and/or lessons. Place all forms and fees (exact cash or check to GCS) in a sealed envelope with your name on it and give it to Ms. Felix before the deadline.

Parents, please complete and sign the appropriate form(s) and make sure they're turned when your student gets on the bus. If your child is renting and taking a lesson (even for a 3 Class Pass rental + lesson), you need to fill out both forms.

A Note About Illness
If a student is too sick to come to school on Thursday, then s/he is too sick to go skiing/ boarding on Friday! If you know that your student will not be able to attend due to illness, please text Mrs. Newberg on her cell phone (see the Home Page for contact info) as soon as possible. If you have paid for lift tickets/rentals/lessons, we may be able to apply a credit to next week’s trip as long as we have been notified in advance.
