Annual Notice and Opt-Out Exemption Forms for Statewide Tests:

Gervais School District hopes to administer state assessments to ALL eligible students. While it is the parent/guardians' right to request opt-out/exemption from Oregon ELA and math testing for their student(s), we encourage parents to allow their students to test. Students do not "pass/fail" these tests. The tests simply indicate to us where and how we need to improve instruction so that ALL students succeed.
