Kindergarten Requirements

 Medical Records Needed for Entry into Kindergarten: 

❏ Immunization records with dates of all immunizations http://www.cdc.gov/vaccines/schedules/easy-to-read/child.html

❏ Copy of student' 5 year physical exam, or an appointment for one if the student has not yet had this appoinment

❏ Completed Student Medical History Form 

❏ Documentation of lead screening
