Health Requirements
Physical Exams are required for Pre-K, Kindergarten, 6th grade, 9th grade, all new students, and every year for sports participation. These must be on the IL Child Health Exam Form for the required grade level physicals.
Dental Exams are required for Kindergarten, 2nd grade, 6th grade, and 9th grade.
Eye Exams are required for Kindergarten and any student entering the district and/or state of Illinois for the first time.
VACCINE PREVENTABLE DISEASES
Immunization protects against the following 10 serious diseases, which can cause disability and death. These diseases used to strike thousands of children each year. Today there are relatively few cases, but outbreaks still occur each year because some babies are not immunized.
