Because case volume is lower in rural areas, it's important to be proactive and take opportunities as they come. Introduce yourself to people and request to be notified if possible when things come up (procedures, important cases, etc.). We want this rotation to be high-yield.
Before you go
Contact your clinic for rescheduling or cancelling of clinic as soon as this elective is approved. Indicate status of clinic on your Elective Request Form.
Attend any specific conferences or meetings as directed by your onsite Attending. You are excused from IM Morning Report, Noon Conference and Grand Rounds during this rotation.

Housing/travel/paperwork
If housing is needed, work with Med-Peds team on housing preferences and check in/out dates.
Submit any additional paperwork requested by sites back to the Return and/or the Med-Peds Project Manager as requested (gme.return@hsc.utah.edu/hailey.haffey@hsc.utah.edu) at least 2 weeks before rotation.
Save all mileage receipts and submit mileage reimbursement form within 20 days of Rotation completion.
Complete Rotation evaluation surveys.
Becoming an Indian Health Services (IHS) employee and credentialing

Note: First you need to be contracted and credentialed through the UoU in a visiting role. Your salary comes through the UoU as does your malpractice insurance. However, you will need to also be employed and credentialed through IHS, which is a detailed process.
1. Credentialing packet:
- Notify Janet Blacksheep, Janet.Blacksheep@ihs.gov. She will send you the documents or refer to our Credential Packet Folder #1-9.
- Get a copy of your immunization records (including recent PPD if you have it).
- Documentation of N-95 mask fitting certificate if available.
2. Fingerprinting
- Two blank cards will be mailed to your address.
- Go to the local police department. Occasionally, a FedEx/UPS place can usually do it for you. Keep your receipt.
- Mail fingerprints as directed by the Rural Pathway Project Manager or Program Director.
If you are unable to fulfill this requirement, you can get your fingerprints done in Chinle. Be sure to arrive before 5 pm and inform Janet that you will need fingerprinting done, so they can arrange it with Human Resources prior to your arrival.
3. NPI number
After residency, you need to change your status from resident/student to provider. Shauna Maxwell will plan to do it for you if you give her your username/password, but if not you can go to: https://nppes.cms.hhs.gov/NPPES/StaticForward.do?forward=static.npistart and update your “taxonomy” yourself.
IHS requires this so that they can be billed for your work.
4. DEA license
- Your DEA will be under the same as the Locum Tenens providers and there is nothing you need to do for it.
5. Work email
Will set-up at check-in.

6. Pre-Orientation/Check-in work:
You will have a day to go through the check-in process at Chinle; this requires meeting with Jocelyn Lewis or Janet Blacksheep first. You will receive a hand-out requiring you to go to different departments around the hospital to “check in” (i.e. Housing, Human Resources, Infection Control, etc.), as well as online computer training.
