11th grade physical exam form
All students entering 11th grade are required to have a physical, according to the Pennsylvania Department of Health and Department of Education. Any physical examination completed within 12 months of the first day of school will be accepted. All sports physicals done at school within the past year will also be accepted (the school nurse will obtain a copy from the athletic directors).
Please submit for to Mrs. Lanschcak in the high school nurses office.
