New Students

Students New to North Carolina

All students who have never been enrolled in a North Carolina public school are required by law to have a health assessment completed within 30 days of enrollment. The form can be found here (Spanish Version).

Kindergarten Enrollment 

Enrollment is completed online through Scribbles.  All kindergarten students must have their NC Required Immunizations and a North Carolina Health Assessment Form (Spanish Version) completed and uploaded to the Scribbles site by the 30th day of school/enrollment.

7th Grade Required Immunizations

All 7th graders in NC schools must have their 7th grade immunizations completed by the 30th day of school/enrollment.

1. MCV-meningococcal

2. TdaP-tetanus, diptheria, acellullar pertussis

12th Grade Required Immunizations

All 12th grade students are required to have a Menigococcal (MCV) booster vaccine by the the 30th day of school/enrollment.
