New Students
New Student Requirements
Illinois School Physical done within the past 365 days of the start of school.
For students transferring from an Illinois school, a request should be made to have the previous school send health records to your child's current school as this requirement may have been met. If physical and immunization records are not included in the Illinois school transfer information, parents must provide a new Illinois physical.
Up-to-date immunizations
Includes the following series: Dtap, Tdap, Polio, Measles, Mumps, Rubella (MMR), Haemophilus influenzae type B (Hib), Pneumococcal Conjugate (PCV 13), Hepatitis B, Varicella, Meningococcal (MCV) . If unsure which vaccinations your child needs, please contact primary care physician's office, local health department or the school nurse.
Religious Waiver: Must be completed and signed by parent/guardian AND physician. See "Action Plans and Health Forms" for required form.
Medical Waiver: Must be completed and signed by the physician.
**Students who register before the first day of the school year, must present required health records or proof of a scheduled appointment by the first day of attendance. The appointment must be scheduled within 30 days of the first day of school.**
**Students who register after the first day of the school year, must present required health records or proof of a scheduled appointment by the first day of attendance. The appointment must be scheduled within 30 days of registration.**
Vision Examination: Proof must be submitted before October 15 of the current school year that the student received an eye examination within one year prior to entry.
Dental Examination: For students in Kindergarten, Second, Sixth, or Ninth grades. Proof must be submitted before May 15 of the current school year that the student received a dental examination within 18 months prior to entry.
