The following forms must be completed and turned in prior to the first day of tryout clinics. All of these forms were included in the tryout packet you received before or at the parent meeting. If you have any questions, please contact Dee Doss at dee.doss@pellcityschools.net or call 205-338-2250.

NFHS Sportsmanship Program
If you don’t already have an account with NFHS, you will need to create one (free). Prospective players, it is recommended that you use your student email through Pell City School System.
When it asks for your sport, choose SPIRIT.
Choose Pell City High School for JV / Varsity or Choose Duran South for Jr High.
Order the course. You will see a pop-up--”this course will be completed by:” Select "myself"
Select state - Alabama. The course is free.
Proceed to Check-out.
Congratulations. You are in the program.
Once completed, print certificate, and submit to your coach.
PRE-PARTICIPATION / Physical Form

We will have this on file if you have previously cheered for Pell City School System. It will be valid through the summer. IF you did NOT cheer for PCSS, you must have your Doctor complete and sign the physical. Forms may be found and printed by clicking on the 'AHSAA Preparticipation Physical Form' button under links. (NOTE: the form must say REVISED 2018. )
If this is not turned in BEFORE the first day of clinics, you can not try out.

Each student-athlete must have an account on Dragonflymax.com for PELL CITY HIGH SCHOOL (Code 8TYQFD).
If you are Jr High, DURAN uses the code (KDZRGR).
If you are a previous Jr High athlete entering the 9th grade, you will need to add PCHS to your account.
