Back to School Forms

• VacTraK This form allows the school nurse to access the Alaska VacTrAK system where immunization information is stored.

• Physical Exam Form Required upon initial entry to KPBSD schools.

• Health History Form Will be completed upon entry to school, it also authorizes medical treatment. Please keep up to date.

Medication at School

• Over the Counter Medication Form Must be completed for over the counter medication like Tylenol, Ibuprofen, Tums, cough drops, etc.

• Short Term Medication Form Must be completed for prescription medication to be administered short term (less than 2 weeks). For instance, antibiotics that are to be given multiple times a day.**

• Long Term Medication Form Required to be completed by the parent or guardian & prescribing physician. This is for a daily medication that is not covered by the Allergy/Anaphylaxis Asthma Action Paperwork, Seizure paperwork.

** Please note all prescription medication that is brought to school must have a label from the pharmacy intact. Please do not bring expired medication. If there is a supply issue from the pharmacy please notify the nurse.
