Hays CISD requires a physical to be completed on or after April 1st, 2024 prior to participation in athletics. Complete the medical history on the front, sign the bottom of the page, and have a physical evaluation completed by a physician. Physicals MUST be turned into the Athletic Trainers. If you will be participating in athletics AND band, turn the form into the Athletic Trainers prior to submission to band.

ALL PHYSICALS ARE DUE BY THE START OF ATHLETICS IN AUGUST. PHYSICALS MUST BE COMPLETED PRIOR TO PARTICIPATION. NO PHYSICAL, NO ATHLETICS. 

UIL and Hays CISD require the completion and approval of all RankOne Forms prior to participation in athletics. All forms MUST be completed with a parent. Full signatures are REQUIRED, initials WILL NOT BE ACCEPTED. All forms will need to be approved by the Athletic Training Staff prior to participation.
