Forms

All projects require form 1, 1a and 1b. Additional forms may be required depending on project type and design.

Forms must be completed and signed PRIOR to beginning the project. Forms should be submitted to the OMS Science Fair Coordinator by December 6th.

Use the ISEF Rules Wizard to determine if your project idea requires additional forms or approvals.

NOTE: If you project involves any of the following, you need to use the ISEF Rules Wizard and speak to the Science Fair Coordinator immediately.

Human participants

Vertebrate animals

Recombinant DNA

Microorganisms

Human or animal tissue

Hazardous chemicals, activities, or devices

Student-designed invention, app, or prototype
