PLEASE NOTE:
FPS utilizes the State of Arkansas registration process that provides space to upload documents that
Fayetteville Public Schools does not require for registration.
YOU WILL ONLY NEED TO UPLOAD THE DOCUMENTS LISTED BELOW.  

2. Have these documents ready to upload when enrolling your child. You can upload the necessary registration documents during the registration process.

• Proof of Address (current lease agreement or gas/electric/water bill dated within the last 30 days)

• Prior to a child's admission to an Arkansas public school, the parent, guardian, or responsible person shall provide the school district with one (1) of the following documents indicating the child's age:

  • • • A birth certificate

      • A statement by the local registrar or a county recorder certifying the child's date of birth

      • An attested baptismal certificate;

      • A passport

      • An affidavit of the date and place of birth by the child's parent or guardian

      • Previous school records

      • A United States military identification

• Immunization Records

• Social Security Card 

• Proof of a physical exam dated within the last 2 years

3. After you submit your child's enrollment, you will receive one additional form Student Enrollment Form via your provided email that you will need to complete and submit.

4. WHAT'S NEXT?

Once your enrollment is completed, documents have been uploaded, and the additional Student Enrollment Form submitted,  the child's record will be processed and reviewed.  Your child's school will contact you when the process is complete, or if there are questions.
