日頃より新型コロナウイルス感染拡大防止にご協力くださりありがとうございます。首都圏の新規感染者は高止まりの状況のため、引き続き感染拡大防止に努めて頂きますようお願いいたします。
特に所内での感染拡大を防ぐため、発熱（37.5℃以上）がある場合や、下記の事例に該当する場合は直ちに研究室主宰者に連絡するとともに、自宅待機を徹底ください。また、感染の可能性がありましたら早めに環境安全チーム（kankyoanzen.iis@gs.mail.u-tokyo.ac.jp）にご連絡頂くようお願いいたします。具体的には、以下のような事例が対象となります。
Thank you for your continued cooperation in preventing the spread of the new coronavirus. As the number of newly infected people in the Tokyo metropolitan area remains high, we would like to ask you to continue your efforts to prevent the spread of infection.
In particular, in order to prevent the spread of infection within IIS, if you have a fever (37.5℃ or higher) or one of the following cases applies to you, please make sure to immediately inform the host faculty member of your laboratory and stay home. In addition, if there is a possibility of infection, please contact the Environment, Health and Safety Section (kankyoanzen.iis@gs.mail.u-tokyo.ac.jp) as soon as possible. Specifically, the following cases are applicable.
本人の発熱（37.5℃以上）が数日続くとき You have a fever (37.5℃ or higher) that lasts for several days
本人が保健所から陽性の認定を受けたとき You have been certified as positive by the public health center
同居する家族等が陽性のとき A family member or someone living with you is positive
陽性者と密に接触したとき You have had close contact with a positive person
本人が保健所から濃厚接触者の認定を受けたとき You have been certified as a close contact by a public health center
本人が感染者・濃厚接触者と接触したとき You have been in contact with an infected person or someone who is a close contact
同居する家族等が保健所から濃厚接触者の認定を受けたとき A family member or someone living with you is certified as a close contact
報告頂く際は、以下の情報を漏れなく記載頂きたく思います。
When reporting, we would like you to provide the following information without any omission.
氏名（漢字、よみがな） Full name (please provide "kana" indicating pronunciation)
所属・職名（学生・特任教員の場合には所属研究室の教員名も）Affiliation and job title (if you are a student or a project faculty
member, please include the name of the host faculty member of your laboratory)
生年月日 Date of birth
共通ＩＤ／学生証番号 Common ID/student ID number
住所（管轄の保健所を把握するため） Address (to identify the health center with jurisdiction)
電話（内線） Telephone number (extension)
電話（緊急連絡先、携帯など） Phone (emergency contact, cell phone, etc.)
連絡先メール Contact e-mail
現在の状況（陽性／陰性、症状） Current status (positive/negative, symptoms)
接触・体調履歴（２週間分、自宅待機の日数算出のため） Contact/physical condition history (for two weeks, for calculation of the number of days to stay at home)
最後に入構した日 Date of last entry to campus
接触した人のリスト、同居する家族等構成 List of people with whom you have had contact, including the composition of family members living with you.
保健所や医師からの指示内容（待機期間など）Details of instructions from health centers and physicians (e.g., waiting periods)
なお、新型コロナウイルス感染症に罹患したとき、自宅療養・療養解除の基準は以下の通りとなります。If you get infected with the novel coronavirus, the criteria for ending isolation ・ self-quarantine in the event you tested positive are as follows.
療養期間は保健所・医療機関から指示があった場合はそれに従ってください。Make sure to follow the instructions from the public health center when they contact you.
保健所・医療機関からの明示的指示がない場合は下記の国の指針を目安に自宅療養・療養解除を行ってください。Even if you do not receive explicit instructions from the public health center,
【症状がある場合】発症日＝症状が出現した日から、10日以上かつ症状軽快後72時間経過後（または症状軽快後24時間以上空けて 2回PCR等の検査を行い、陰性だった場合）に療養解除となります。 [If symptomatic] The patient can be cleared of isolation without another test if they meet the following requirements: 1) 10 or more days since the first day with symptoms and 2) 72 hours since your recovery.
【症状がない場合】検体採取日から７日経過後に療養解除となります。[If asymptomatic] 7 days after the date of sample collection.
【無症状者が途中症状が出た場合】 当初無症状の人であっても、途中で症状が出現してしまったら、発症から10日間は感染性があるとされているため、発症日が起算日となります。[Cases in which an asymptomatic person develops symptoms] 1) 10 or more days since the first day with symptoms and 2) 72 hours since your recovery.
