• Denton Health Form (consent for screenings): In alignment with Senate Bill 12, Denton ISD now requires parent or guardian consent for students to participate in state-mandated health screenings. This fall, our campus nurses will conduct Hearing, Spinal, Acanthosis Nigricans, and Vision screenings (when appropriate, a vision photo screener may be used) following the guidelines of the Texas Department of State Health Services (DSHS).

• Parent Informed Consent for School Based Counseling and Support Services:  In Denton ISD, we are committed to supporting your child’s academic success and overall well-being. As part of our comprehensive school counseling and support services program, students may have access to a certified school counselor and/or school-based support staff.
