DISD instrument lease form

Please print and turn this form in with the first payment. No student will be issued a school instrument until this form and the first payment is turned in. If you are a fee wavier student (free/reduced lunch) you also need to fill out and turn in the fee wavier form and a copy of the lunch status notification that you receive from DISD.
