This assignment pertains to DNP Essential I, Scientific Underpinnings for Practice, because it makes the DNP student critically evaluate a theory that would provide the most benefit for implementation of their DNP project. DNP Essential I embodies the understanding and applicability of scientific bases for implementation and evaluation of organizational changes.
COMPLEXITY THEORY
When looking at healthcare system change and implementation of evidence-based practice changes, the Complexity theory could apply. Complexity theory first arose in 1984 and has since evolved just as our healthcare system has (Chandler, 2016). It is essential to understand the organizational structure of primary care practices prior to implementing any change (Chandler, 2016). The complexity theory is driven by the need to differentiate order and design (Chandler, 2016). This theory excludes “intentionality” whereby transforming the theory into motivation and shared decision making (Chandler, 2016). The complexity theory explains order, structure, and behavior patterns (Chandler, 2016).
The complexity theory would be useful for any healthcare organization to utilize prior to implementation of any process changes. Specifically, the complexity theory would be useful in identifying barriers among providers and cervical and breast cancer screening rates. Identification of why providers have low cervical and breast cancer screening rates among their female patients, essentially could lead to process changes thus allowing for identification of these cancers in early stages. The literature reports several barriers for providers which attribute to the low rates of screening, including lack of resources, lack of or ineffective system provider reminders, lack of knowledge or understanding of new screening guidelines, and patient noncompliance.
Using complexity theory, provider and patient behaviors can be observed, organizational structure and policies reviewed, and processes studied.
